These services are offered both online and offline.

Qualifications and Credentials: You understand and acknowledge that the doctors, physiotherapists, nutritionists, psychologists, health coaches and other allied healthcare professionals associated with Painzio Health Pvt. Ltd., possess the necessary qualifications, certifications, and experience to provide the services. However, Painzio Health Pvt. Ltd. cannot guarantee the effectiveness or outcome of any particular treatment or intervention.

Online Services:

1. Privacy and Security: You understand that online consultations and communications may involve the transmission of your personal health information over the internet. Painzio Health Pvt. Ltd. takes reasonable measures to ensure the privacy and security of your information but cannot guarantee absolute confidentiality.

2. Technical Requirements: You are responsible for ensuring that you have the necessary technical requirements (e.g., stable internet connection, compatible devices) to access and participate in online consultations or programs.

Responsibilities and Limitations:

1. Personal Responsibility: You acknowledge that you are solely responsible for your own health and well-being. The services provided by Painzio Health Pvt. Ltd. are not for an acute or emergency medical conditions and Painzio Health Pvt. Ltd. would recommend finding a substitute for professional medical advice, diagnosis, or treatment in case of immediate acute pain or emergency medical condition. Under such conditions, you should consult with a qualified healthcare professional for any specific medical concerns.

2. Limitations of Services: Painzio Health Pvt. Ltd. provides medical and general advice based on the information provided by you. In case you have informed something inaccurately pertaining to your specific circumstances, the effectiveness of the services may vary.

Confidentiality: Painzio Health Pvt. Ltd. is committed to protecting your privacy and will handle your personal and health information in accordance with applicable laws and regulations. However, there are certain circumstances where Painzio Health Pvt. Ltd. may be required to disclose your information, such as complying with legal obligations or court orders.

Medical Release: A key part of maintaining good health is taking full responsibility of monitoring your needs. It's important that you understand:

1. The advice of your coach cannot replace the advice of a trained medical doctor.

2. It's your responsibility to work directly with your physician/doctor before, during, and after seeking coaching.

3. If you choose to be coached without the prior consent of your physician/doctor, you agree to accept full responsibility for your decisions and to hold harmless Painzio, its agents, officers, and employers and any affiliated companies from any liability with respect to injury to you or your property arising out of or connected with your use of the information discussed.

Sensitive Information: Painzio Health Private Limited (Painzio) processes data on behalf of you and your care team. It’s important to us that you understand what we (Painzio) do with the data you provide.

1. Painzio does not share or sell your information. The only people who will have access to your data are you, your care team (which would include your Doctor, your Physiotherapist/s, your care manager and Painzio's support team). You also understand that the care team including the care manager and support team might be non-medical person/s)

2. It is up to you to decide what and how much data you provide to your care team. You can share everything with your care team or provide very little information, depending on how you and your care team choose to work together.

3. The information you can choose to share includes answers to questions in this form, any uploads and data shared during consultations or with your care team. Only you and your care team can access this information, unless you specifically request otherwise.

Changes to Terms and Conditions: Painzio Health Pvt. Ltd. reserves the right to modify or update these terms and conditions from time to time. Any changes will be communicated to you through appropriate means.

Consent for Treatment and Participation: By signing this form, you confirm that:

1. You voluntarily consent to receive the services offered by Painzio Health Pvt. Ltd. and understand the nature of the services provided.

2. You acknowledge that the results of any treatment, intervention, or advice provided by Painzio Health Pvt. Ltd. are not guaranteed.

3. You authorize Painzio Health Pvt. Ltd. and its healthcare professionals to collect, use, and disclose your personal and health information for the purposes of providing the services and for administrative and legal requirements.
